Technical Assistance

Our experience in the health and wellness field drives our direct technical assistance for chronic disease prevention and self-management programs. By providing programs with support, research, education, and sustainability resources, it is hopeful that Pennsylvania will have a strong network of education providers to ensure access to evidence-based programs for all those with and at risk for diabetes and other chronic diseases.

Diabetes Self-Management Education

PA CCI’s technical assistance (TA) platform offers both self-directed and guided learning opportunities accompanied by access to a technical assistance team to supplement the ADA and AADE support. Technical assistance participants will:

 

1) apply for scholarships to cover the cost of certification application fees;

 

2) access the PA CCI website to participate in continuing education webinars, post questions and resources, view trending news and research, connect with peers, and obtain resources to support DSME programming, reimbursement, and program marketing; and

 

3) engage with the HPC technical assistance team to address DSME-related issues.

Diabetes Prevention Program

The Diabetes Prevention Program (DPP) was a major clinical research initiative seeking to discover whether moderate weight loss through diet changes, along with physical activity would delay or even prevent type 2 diabetes.  The DPP proved that a weight loss of 5-7% body weight combined with 150 minutes of physical activity a week reduced the risk of developing type 2 diabetes by 58%.  This study has been transformed into a community-based lifestyle change program facilitated by a trained Lifestyle Coach.  The goal is to use a motivational group environment to reinforced healthy habits. PA CCI is increasing the number of DPP sites in Pennsylvania through direct technical assistance and funding of high priority sites and the training of Lifestyle Coaches.
